Medical Case Manager

The Case Manager Assistant will support case managers in overseeing patient cases, focusing on medical treatments for Traumatic Brain Injury (TBI), Neurological conditions, and other medical services. The role involves handling inbound and outbound calls, scheduling appointments, conducting patient intakes, and ensuring timely follow-ups with patients.


Key Responsibilities:

  • Handle inbound and outbound calls with patients, healthcare providers, and other stakeholders to ensure smooth communication and coordination of care.

  • Conduct patient intakes, including collecting necessary medical and personal information, and inputting data into the system.

  • Set appointments for patients, coordinating with healthcare providers and ensuring patients are scheduled according to their treatment plan.

  • Follow up with patients to ensure they attend their appointments and receive required treatments, addressing any concerns or issues that may arise.

  • Provide information and assistance to patients regarding their treatments, answering any questions related to their appointments or health plan.

  • Maintain accurate and up-to-date records of patient interactions, appointments, and treatments.

  • Collaborate with the case manager and other team members to ensure proper case management and patient care.

  • Monitor patient progress and ensure that follow-up tasks are completed within appropriate timeframes.

  • Assist with administrative tasks such as filing, data entry, and report preparation.

Skills and Qualifications:

  • Excellent verbal and written communication skills, especially for handling patient inquiries and follow-ups.

  • Strong organizational skills and attention to detail.

  • Ability to handle confidential information and maintain patient privacy.

  • Previous experience in a healthcare setting, particularly in patient intake or case management, is a plus.

  • Comfortable making phone calls and interacting with patients, healthcare providers, and other team members.

  • Ability to multitask in a fast-paced environment and manage time effectively.

  • Proficient in using computer systems and office software for scheduling and data entry.

  • Knowledge of medical terminology, particularly in relation to TBI (Traumatic Brain Injury) and neurological conditions, is a plus but not required.
